China Chef, a takeaway at 56 West Street, Fishguard, Pembrokeshire was given the score after assessment on November 7, the Food Standards Agency's website shows.
It means that of Pembrokeshire's 96 takeaways with ratings, 49 have ratings of five and none have zero ratings.
