Unpaid carers who are able to support a loved one at home to avoid a hospital admission or help facilitate early discharge could be eligible for a grant of £200 per week, for up to six weeks.
Pembrokeshire County Council has been awarded funding for the Hospital Discharges Carer Grant (one payment per household).
The grant will help people recover at home out of the hospital setting and reduce pressures on the NHS over the festive season and winter.
Unpaid carers living in Pembrokeshire are eligible if they are supporting someone who:
• Would need to be admitted to hospital without their support
• Is medically well enough to leave hospital but are unable to be discharged because extra support is required, which is not available from care agencies at the time of leaving hospital.
Hospital and/or Social Care staff will identify individuals and families who may benefit from the grant and discuss the opportunity with them.
If your loved one is in hospital and you think you are eligible for the payment please speak with the ward manager, who will ask one of the joint discharge team to explain the details.
